Step 1
Simply find your own IP Adress or Default Gateway.
To find it, try going to the Ps4 and go to Settings-Network-View Connection Status and that's where it could be found.
If that doesn't work than simply go on your Laptop/Computer/Desktop and press the Windows Key + R and it should open the Command Prompt, or click on the start bar ( its the search bar on the bottom left corner of the screen ) and type cmd.
Once you have opened it, simply press on the space bar once and type ( ipconfig ) then press enter and the following info will display.

Step 2
Now that you have your Default Gateway Address, simply copy and paste it onto google. There you should be connected to your router, there should be a page called " NetWork " (or could be named something else) once you go on there, it will show all the addresses of the devices that are/were connected to the router. Find the Ps4 address and copy it. After you have found the Ps4 address and have copied it, go to a page called "Security" or "Port Forwarding" ETC.
Step 3
For the type of game that you want to select to port forward, you will first need the TCP and UDP of it. To find it go to the website portforward.com and search the name of the game and even your router's name and you will find the TCP and UDP. Go back to the router page and copy and paste it onto there.
Example: Fortnite
the TCP and UDP of Fortnite- Ps4 is
TCP: 1935, 3478-3480
UDP: 3074, 3478-3479
1935 is the start port, so simply just copy and paste in into the start and end port and put the type TCP and then you will have to put your ps4 address that you found earlier and copy and paste it and save.
Now 3478-3480 are the ranges, 3478 being the start and 3480 being the end then add the Ps4 address and save it.
For the UDP repeat the exact same process like the TCP.
There might be an error when doing the ranges on UDP since the range of the TCP is the same, just simply remove the range box and go to the range box of the TCP you did earlier and instead of putting the type as TCP, put it as Both.
If another game only has a set of numbers, for example, Modern Warfare which has:
TCP: 28960
UDP: 28960
simply apply the TCP with 28960 start-end port, and since the TCP and UDP have the exact same numbers just apply the type to both.
Example: Fortnite
